When sending a request for a survey, please include the following information:
- Goal: What are your goals for the data you gather?
- Sample: Who will you need to gather information from?
- Initial Content Development: What do you want to ask?
- Method/Type: Focus Groups, Interviews, Survey by Email, tablet, paper-and-pencil, etc.
- Timeline: When do you need the information/results back?
